How do I report a fatality to DAN?
Notification of a fatality should be made by calling DAN Medical Information Line at 1-800-446-2671, or by contacting DAN Medical Services.

Copies of the current Fatality Report Forms can be obtained by contacting DAN Research at 1-800-446-2671 x237 (+1-919-684-2948 x237).
How can I obtain a copy of the annual Report on Decompression Illness, Diving Fatalities and Project Dive Exploration?
Copies of this report can be ordered online or by placing an order at 1-800-446-2671 (+1-919-684-2948). DAN Members may also download the most recent report at no cost.
How can I obtain a copy of fatality reports from DAN?
A valid legal authorization is required to obtain records from DAN. In the case of a fatality, a subpoena from NC District 14 or the Middle District is required. If you are the executor of the decedents estate, you must show legal prove of this to DAN. All legal requests should be submitted to DAN's Custodian of Records and all must be approved by the DAN legal counsel.
